The Bottle of Life program provides free informational kits that aim to make it easier for emergency responders to gather important info during a medical emergency when the patient is otherwise unable to communicate. It is designed to speak for you when you cannot speak for yourself. This important personal medical information form can assist emergency personnel in administering the proper medical treatment.
The kit includes a simple paper form which can be completed by residents. The form identifies any ongoing health issues, allergies to medications, what prescriptions are being taken, emergency contact information. This form is then inserted into a small bottle that looks like a prescription container from a pharmacy that has a brightly colored sticker indicating that it is a "Bottle of Life". The bottle is then placed somewhere visible inside of the household refrigerator. A similar sticker is then placed on the door of the refrigerator itself.
Kits are free to the public and are available to be picked up at Moapa Valley Fire District Headquarters located at 3570 N Lyman St, Logandale, NV 89040.
